Complete these steps in advance.
- Complete your digital intake forms (we'll email a link before your visit)
- Gather your insurance card - front and back
- Bring a photo ID (driver's license or passport)
- Write down all current medications and dosages, including supplements
- Note any allergies (medications, latex, adhesives)
- Gather prior imaging: X-ray CDs, MRI reports, or radiology printouts
- Write down when your symptoms started and what makes them better or worse
- List the activities your foot/ankle problem is preventing you from doing
- Obtain a referral from your primary care doctor if required by your HMO plan

Arrive prepared and comfortable.
- Wear or bring the shoes you wear most often - athletic shoes, work boots, or dress shoes
- Wear loose, comfortable socks that are easy to remove
- If you have lower leg or ankle concerns, wear shorts or easily rolled-up pants
- Arrive 10 minutes early to complete any remaining paperwork
- Park in the free lot at 1500 W 3rd Ave, Suite 120, Grandview Heights
- Bring a family member or support person if you'd like - they're always welcome
- Be ready to describe your pain on a scale of 1-10 and where exactly it's located

What should I bring to my first podiatry appointment?+
Bring your photo ID, insurance card (front and back), a list of current medications and allergies, any prior X-rays or imaging, and the shoes you wear most often. Complete our digital intake forms in advance if possible - we'll send them before your visit.
What shoes should I wear to a podiatry appointment?+
Wear or bring the shoes you use most often - your everyday athletic shoes, work boots, or dress shoes. Dr. Golan often examines footwear as part of the biomechanical assessment, so the shoes you actually live in give him the most useful information.
How early should I arrive for my first podiatry appointment?+
Plan to arrive 10 minutes before your scheduled time. If you completed your intake forms online in advance, check-in is quick. If not, arriving early gives you time to complete paperwork in the office.
Can I bring someone with me to my podiatry appointment?+
Absolutely. Family members, spouses, and support persons are welcome at all appointments. If you prefer a Spanish-speaking consultation, Dr. Golan and staff provide the full visit in Spanish - no translator needed.
Do I need a referral for my first podiatry appointment?+
In most cases, no. Most insurance plans allow direct access to a podiatrist without a primary care referral. HMO plans may require one - check your insurance card or call (614) 328-5561 and our staff will help you verify.
